What is acne?

Acne is an inflammatory skin disease that most often appears on the face, neck, décolletage, shoulders and back. They can manifest in the form of blackheads – comedones or red and yellowish pimples – papulo pustules.

How does acne occur?

The appearance of acne is caused by increased activity of sebaceous glands that produce excess sebum, which, along with disturbed peeling of epithelial cells, leads to the formation of open blackheads – comedones that grow and cause blockage of the outlet channels of the sebaceous glands by creating plugs, which form closed blackheads. The settlement and multiplication of bacteria on such terrain intensifies the inflammatory process, accompanied by redness and the appearance of pus.

Who gets acne?

There are different reasons for the appearance of acne and they can manifest at different ages.

Juvenile pubertal pimples

About 80% of the world’s population between the ages of 15 and 25 suffer from acne, and the first ones appear in adolescents around the age of 11 and last into the teenage years in 90% of cases.

Acne in adults

However, even the elderly population is not spared from acne, so 3% of people over 40 still have this problem.

Types of acne

The most common name for acne is pimples in the general population. However, it could be just one of the stages in the development of acne. They most often appear on the face, neck, shoulders and back.

Types of acne (morphological division)

Depending on the lesions that dominate the clinical picture, acne is divided into:

  • open and closed comedones,
  • papules,
  • pustules,
  • nodules and
  • pseudocysts

Open and closed comedones

This is the mildest form of acne, without inflammation. These forms do not result in scarring.

Open comedones

Popularly known as blackheads or “black dots” in the pores.

Closed comedones

These are hemispherical pale nodules with a diameter of about 2 mm that appear on oily skin with enlarged pores.

Papules

They represent a medium-severe form of acne. They are caused by inflammation of closed comedones and look like a reddish nodule, without pus.

This form can leave scars.

Pustules

Also a medium-severe form of acne caused by closed comedones that tends to leave scars. These are bubbles filled with pus – suppurations.

Knots

They represent the most severe form of acne. Comedones, papules and pustules are also present with him. The inflammatory process progresses and affects the deeper parts of the follicle. They leave scars.

The dominant lesions are:

  • nodules and
  • cysts
Nodules

Hard painful nodules of a spherical shape – diameter greater than 0.5 cm. They most often appear on the back and shoulders.

Cysts

They are located deeper in the dermis, often inflamed and painful.

This form occurs more often in men.

Acne throughout life

Although they most often occur in adolescents, acne can occur at any age, even in babies.

Depending on the age at which they appeared, acne is divided into:

  • Acne neonatorum
  • Infantile acne
  • Acne vulgaris and
  • Acne tarda.

Acne neonatorum

They appear after birth, in most cases on the cheeks of babies, and usually disappear around 3 months of age. It is often the case that they do not need to be treated, but pass spontaneously if adequate care is used. They rarely require treatment.

Infantile acne

They have the same characteristics as neonatal acne, but appear on the skin between 3 and 12 months of age.

Acne vulgaris

Juvenile or pubertal pimples that appear in adolescence between the ages of 15 and 18. This is also the most common form of acne affecting the face and upper body. They often leave scars if left untreated.

Acne tarda

They occur more often in women after the age of 25 and on the chin and jaw. However, they can also affect the entire face, neck and upper body. Like Acne vulgaris they can leave scars.

Types of acne according to severity of symptoms

Acne can be mild, moderate or severe. With that in mind, they can be divided into:

  • Acne comedonica,
  • Acne papulopustulosa,
  • Acne conglobata and
  • Acne fulminans.

Acne comedonica

This is the mildest but most common form of acne. It is characteristic for them that comedones and only a few pustules are visible on the skin.

As we have already mentioned, they can be open or closed, and their cause is excessive activity of the sebaceous glands and an excessive amount of sebum. Dead skin cells, dirt and grease collect in the blocked pores, creating a visible plug and the face becomes rough to the touch.

People with naturally enlarged pores are more prone to this acne.

Acne papulopustular

They belong to the moderate form of acne, characterized by papules and pustules. Papules often turn into pustules, from reddish nodules to purulent blisters if left untreated. They often lead to scarring.

Acne conglobata

This form belongs to severe acne and represents a serious dermatological problem, but it is not common in the world population.

They are a combination of nodules (hard lesions deep in the skin) and cysts (acne deep in the skin filled with pus), which is why they are also known as acne nodulo-cystica.

Acne fulminans

The rarest form of acne. It occurs more often in men. They are also known as acute febrile ulcerative acne.

Traditional acne treatment

Treatment of acne is a very slow and long-term process that can last up to a year when it comes to severe forms.

During the treatment period, it is necessary to treat the skin with antibacterial agents (soaps, lotions and creams) and local antibiotics, with frequent oral use of antibiotics.

Hormonal therapy (in women) is also used in more severe cases.

There are several ways to treat acne.

Different traditional ways of treating acne

For the treatment of acne, the following are most often used:

  • antibacterial preparations containing benzoyl peroxide
  • topical antibiotics applied to acne-affected skin
  • oral antibiotics
  • hormonal treatment (for example, contraceptive pills)
  • skin abrasion (removal of the surface layers of the skin, either mechanically or chemically – with salicylic or glycolic acid)
  • phototherapy
  • photodynamic therapy

However, traditional acne treatment with topical preparations and antibiotics can have side effects:

  • skin irritations (burning and flaking)
  • skin photosensitivity
  • dryness of the mouth, nose, skin and eyes
  • diarrhea, colon irritation
